We, the Registrar’s Office of Saint Louis College of the City of San Fernando, La Union envision an excellent records management responsive to the needs of the academic community and other sectors by handling student’s records with utmost accuracy, integrity, confidentiality and orderliness.

  • Attending to varied request of students, graduates and other entity such as request for OTR, diploma, transfer credentials, record verification, course description, medium of instruction, etc.
  • Evaluation of the academic records of students
  • Posting of students’ final grades in their respective permanent records
  • Filing of students’ permanent records
  • Assisting during period of enrolment
  • Correction of name
  • Preparing the class program and teachers’ program
  • Reconstructing lost or missing records

*The student or his authorized representative goes to the Registrar’s Office and apply for the credentials he wants to secure. He must present:

  • A recent valid ID with a picture on it.
  • If representative, an authorization letter and a recent valid ID with picture of the student he is representing.
  • Official Receipt of payment

HOW TO SECURE SECOND COPY OF LOST DIPLOMA:

The former student or his authorized representative goes to the Registrar’s Office and apply for a second copy of the diploma. He must present:

  • A recent valid ID with picture on it.
  • An Affidavit of loss duly notarized.
  • Letter of request for a 2nd copy of the diploma.
  • If representative, an authorization letter and a recent valid ID with a picture of the former student he is representing.
  • Official Receipt of payment of 2nd copy.

*Note: Release of Diploma is up to 2nd Copy only.

HOW TO AUTHENTICATE OFFICIAL TRANSCRIPT OF RECORDS AND DIPLOMA:

The student or his authorized representative submit to the Registrar’s Office at least two copies of his Official Transcript of Records and Diploma and goes to the Accounting Office to pay the authentication fee of:

P150.00 (for Red Ribbon) – that is, with further CHED authentication or P100.00 (for local)

HOW TO CORRECT A NAME:

A student who wish his name to be corrected shall submit to the Registrar’s Office two copies each of the following:

  • Letter of Request of student concerned addressed to the School Registrar
  • Certificate of Live Birth (NSO copy)
  • Affidavit of Discrepancy of:
    • Student Concerned
    • Two(2) disinterested persons attesting to the fact, among others, that the assumed name refer to one and the same person.
  • Official Receipt of payment for the name correction – P50.00
